“School nursing is a specialized practice of professional nursing that advances the well being, academic success, and life-long achievement of students. To that end, school nurses facilitate positive student responses to normal development; promote health and safety; intervene with actual and potential health problems; provide case management services; and actively collaborate with others to build student and family capacity for adaptation, self management, self advocacy and learning.”
This definition was adopted at the National Association of School Nurses Board of Directors meeting in Providence, Rhode Island in June 1999.
PHYSICAL EXAM
Most recent exam must be provided initially
Subsequent exam needs to be within six months of school entry or during the first year
Vision certificate within 12 months of start of school or within 30 days of the start of the school year, with proof of follow-up if child failed

1 dose Varicella or documented history of chicken pox
Documented history of a lead screen

Dear Parents/Guardians,
Welcome to St. Pius X School. As you complete the transfer/enrollment process our office needs updated medical information. All new students entering from out of district are required to have a recent physical and an up to date immunization record upon enrollment, as per state law.
Reminder notices will be sent home to those students requiring immunizations, including boosters for entry into kindergarten and 7th grade.
If you child has a chronic health condition or significant medical needs, please see the school nurse in order to help develop a care plan. Please submit updated doctor’s notes with your child’s diagnosis. Without such documentation we cannot update your child’s health record and meet his/her needs.
Students requiring medications during the school day will to complete the PARENT/GUARDIAN AUTHORIZATION FOR OVER-THE-COUNTER MEDICATION form or the PARENT/GUARDIAN AUTHORIZATION FOR PRESCRIPTION MEDICATION form along with the MEDICATION ORDER from a licensed prescriber. Physician’s orders are not required for over the counter medications. Medication must be sent in an appropriately labeled bottle. Please note that our office will not stock these latter medications; parents must provide over-the-counter medications for their own child.
Per Massachusetts State Law "over the counter" medications may only be given by a registered nurse; non-licensed personnel is prohibited from giving these medicines to a student. No child will receive these medications when the nurse is not in school, on a field trip, or while in the Extended Day Program.
